Address Rural Implications of Health Reform
“This is the moment when we have the resources we haven’t had before to change health services …that is what’s different about today” said one speaker at the 13th Annual Symposium and that note of optimism permeated the entire conference.

A Light Moment at the Conference
Against the backdrop of beautiful Lake Winnepesaukee, conference attendees heard from representatives of HHS, HRSA, and CMS, who brought the latest news on implementation of the Affordable Care Act during the “HHS” half-day session preceding the annual meeting on October 29. The Symposium was structured around the theme of “Connecting Personal Health and Public Health in Rural New England.” Both sessions attracted enthusiastic participants.
Dr. Andrew Coburn, Muskie School of Public Service, University of Maine, said that in this marathon to implement health reform rural health is at the intersection of ACA: “We have a particularly dynamic region, with a natural affinity for working together. New England can be the source for innovation.”
From Kimberly Kleine, HRSA’s Bureau of Clinician Recruitment, we heard that the National Health Service Corps has simplified its application and approval process, and that its appropriation has been reauthorized at $1.5 BN for the next five years.

Gina Capra brought the good news that ACA is providing impressive support for health centers: $9.5 BN for creation of new sites in medically underserved areas and expansion of primary health care services and $1.5 BN for major construction and renovation projects at CHCs nationwide. (Some NE centers have already benefited from this funding).
With the caveat that Congress must appropriate funds for new (or existing) HRSA programs authorized in legislation before they can be implemented, Daniel Mareck, M.D., HRSA Bureau of Health Professionals, outlined new provisions that will be positive for rural areas: redesign of the Medically Underserved Areas (MUA) and Health Professional Shortage Areas (HPSA) designation process; $253M for expanding physician assistant training; funding of Health Information Technology networks.
Echoing the Symposium theme, keynote speaker Tom Morris, Associate Administrator, Office of Rural Health Policy, emphasized that the Recovery and Affordable Care Act investments represent “Good Timing… for Linking Public Health and Personal Health By Focusing on the Areas of Greatest Need… And Where is the Need Greater than in Rural America?” We need to re-engineer the system to do a better job in health, recognizing that rural areas have real challenges, for one, the insurance market is different in rural areas. In fiscal year 2011, HHS is demanding evidence-based outcomes - “our money is like venture capital; we need to collect performance data to show we’ve had an impact,” Morris said.

Dr. Craig Jones
Turning to regional issues, Craig Jones, M.D., Executive Director Vermont Blueprint for Health, reviewed the Blueprint’s history (leadership at the state level is key). What began as a way to manage chronic diseases has evolved into an integrated system of health management - “Bringing system to non-systems; looking at health as a coordinated continuum” is the Blueprint’s focus. Multi-insurer payment reform underpins the Blueprint.
